| Abstract |
Medical successes have resulted in a growing group of children with medical complexities (CMC). In response to the challenges faced by their families, a Transitional Care Unit (TCU) has been developed near the Amsterdam UMC. The TCU serves as a bridge between hospital care and home caregiving, providing a supportive, home-like environment. Here, families can learn essential skills and routines at their own pace under the supervision of healthcare professionals. Parents are encouraged to actively engage in their child’s care, through which they build confidence within a safe setting. The TCU takes a holistic approach and addresses not only the medical needs of the child, but also the emotional and logistical challenges faced by the entire family. By offering tailored support, we aim to improve the quality of life for these families and reduce the strain on the healthcare system. Our ultimate goal is to empower CMC families to adapt to their new reality, allowing them to manage their own lives while maintaining autonomy.
